Essential Duties and Responsibilities: The mobile medical assistant works closely with our Advance Practice Providers to provide in-home clinical assessments of patients with chronic diseases and become knowledgeable of patient-specific health challenges. Provides in-home point-of-care testing to address any gaps in care by utilizing provider-directed Evidence Based disease-specific clinical interventions and strategies. Provides assessment of patients, measures vital signs such as pulse rate, temperature, blood pressure, weight, and height, and performs point-of-care testing including urinalysis, fingerstick HbA1c, and other screenings, and records information on patients’ charts Develop strong partnerships with the healthcare team. Become a strong patient advocate between internal care providers and patients. Work closely with the multidisciplinary team to understand patientxe2x80x99s risk factors and identify and communicate to providers, the social determinants impacting patientsxe2x80x99 overall health status. Maintain patient records. Focuses on cultivating an excellent patient experience Assists with patient education Willing to travel throughout the state of Pennsylvania Requires to travel up to 100% of the time. Must be willing to travel up to a 2.5-hour radius as needed within the state of Pennsylvania xe2x80x93 mileage reimbursement and travel pay offered Education and Experience: Certified Medical Assistant (CMA) required with completion of AAMA or NCMA certification within 90-days of hire COVID-19 vaccination required BLS-Basic Life Support – Certification preferred or required within 90-days of hire Experience with electronic health records preferred Physical Demands Sitting for periods of time Required and stand, sit, kneel, and crouch while performing work Driving to and from work assignments Lifting up to 25 pounds Ability to multitask May be exposed to infections and contagious diseases Licensure Experience: Geriatric experience preferred Experience working in a Medical Practice/ Home Health//Long Term Care /Acute Care/Health Centers/Community-Based Organizations. Skills: Strong written and verbal communication skills. Bilingual preferred. Completion and documentation in EHR with demonstrated competency. Job Type: Exempt – Full-time
