Who are you? You are a caring and dedicated professional who has a passion for providing mental health services. You are looking to make a difference in your local community be understanding that each individual’s path to recovery is unique. by having this understanding, you also have the working knowledge of the mental health, drug, and alcohol treatment system. You’ll be at the forefront of contact with clients that are both requesting and needing nursing services throughout the organization. You will have the willingness to travel between our office location and our inpatient residential facility, both located in Findlay. Under the guidance of the Nursing Supervisor, you will partner closely with clinical service providers, the medical team, and other staff members to understand and address individuals’ needs related to medical services and medication management. You will engage clients with care and support without requiring close oversight. You have a strong passion for learning the engagement phases of services, and the ability to work with clients that have high resistance and denial or impairment due to chronic mental illness and/or substance abuse disorder dependency. You will receive, manage and route phone calls, messages and/or concerns. Maintain relationships with clients and family members by answering medical questions, policies, and procedures. Act as the first point of contact for clients by capturing necessary information to help facilitate medical appointments and ensure that patients receive warm hand offs to providers. Verify patient information by interviewing patient, recording medical history, confirming purpose of visit, vitals, and any appropriate diagnostic testing. Work as part of an interdisciplinary team by partnering with the nursing team and other staff members regarding client needs for follow-up appointments, medication refills, experiencing symptom changes, etc. Assist in maintaining provider caseloads by providing outreach to clients that have not been seen within 90 days of services to re-engage clients via 10 day closing letter and phone call. Coordinate med somatic appointments with appropriate providers at hospital discharge within a 2 week time period to ensure timely medication refills. Complete prior authorizations with MCO’s, Medicaid, Medicare, and private insurances for client medications in a timely manner to help expedite clients access to medication. You have: HSD/GED CMA (Certified Medical Assistant) with previous experience working as a Medical Assistant A valid driver’s license and the ability to maintain auto insurance CPR/First Aid Certification or willingness to obtain one Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR and other online systems, with multiple logins often simultaneously Willingness to submit background check and fingerprinting Preferably: Previous relevant experience working with individuals that have a primary mental health diagnosis and/or co-occurring disorders Who are we?
