Our Comprehensive primary Care clinic is looking for an experienced LPN (Licensed Practical Nurse) or Certified Medical Assistant (CMA) to function as Care Coordinator for Behavioral Health Integration into Primary Care services. Candidate would be responsible for:xc2xa0Entering and analyzing Mental Health screens and Monitoring Information into EMRReviewing Medications, compliance checks and identifying barriers to careCoordinating care with Primary care Providers (on-site)Doing Care Conferences with Mental Health Practitioners contracted with Clinic to provide consultative services.Provide care coordination services with in-house Therapist.Calling patients to obtain mental health data and calling medication recommendations.
