Most hours will be 8a-4:30p, but there will be some evening events. The schedule is adjusted in that case. Depending on the area the Mobile Clinic Operations Coordinator lives in, there could be some driving between 1-2 hours. Mileage is paid for those instances. ChristianaCare is searching for a Mobile Clinic Operations Coordinator. The primary function is to support the daily operations of the Center for Virtual Health Mobile Clinic by providing logistical, technical, and staffing support. Serve as a training resource for Patient Digital Ambassadors throughout the Center for Virtual Health. Communicate, negotiate, and create partnerships with community stakeholders requesting Mobile Clinic Support. PRINCIPAL DUTIES AND RESPONSIBILITIES: Support the preparation of Standard Operating Procedures for all Mobile Clinic activities Reconcile billing errors and data changes with patient financial services to ensure accurate patient payment Develops and designs worksheets/forms, graphs, and charts, cause and effect diagrams and flowcharts) Coordinate the loading and unloading of the Mobile Clinic for events Negotiate event dates, times, and capacity of events with community partners with the clinical lead and operations director Provides project management admin support as directed by project manager and directors Organizes and leads Mobile Clinic Operations meetings Communicates with members of the healthcare team through electronic software, placing messages in appropriate locations in the EHR or software. Maintains Medical Assistant certification to maximize the utility and functionality of the Mobile Clinic Assists in the enrollment of members in a program according to established program guidelines. Reviews par levels, and preemptively orders adequate supplies for Mobile Clinic operations Identifies problems or gaps in community resources that impact outcomes, care or utilization of services and communicates problems or gaps to the PDA. Identifies Mobile Clinic members with Care Gaps/HEDIS related health conditions and assists them in accessing care through scheduling appointments and referral to community resources Reinforces with members the prescribed treatment plan including attainment of goals and identification of barriers to success Collaborates with the members, care givers, physicians, staff, and other health care professionals across the continuum to provide resources, not limited to: implementing program interventions, documenting activities, assisting providers in obtaining authorizations, making follow up phone calls and sending out educational materials and letters Assists members and caregivers with problem resolutions and strategies to reduce barriers to care and increase health care compliance Identifies problems or gaps in community resources that impact outcomes, care or utilization of services and communicates problems or gaps to the Care Coordinator Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors. Maintains clinical competencies to maximize the utility of the Mobile Clinic Performs other related duties as required. EDUCATION AND EXPERIENCE REQUIREMENTS: For Medical Assistants only: Medical Assistant certification by a national organization accredited by the National Commission for Certifying Agencies (NCCA) or the American National Standards Institute (ANSI) required if working in state of New Jersey. Preferred Associate’s degree in a health care related field; Experience working in the healthcare industry with a focus on population health. SPECIAL REQUIREMENTS: Flexibility and emotional stability to work under intense time and volume pressures. Flexible schedule on weekends and non-standard hours. PHYSICAL DEMANDS: Ability to ambulate within an office setting (walking, stairs, etc.) Occasional sitting, standing and lifting loads of 5-10 pounds. WORKING CONDITIONS: Mix of working in underserved communities outside in variable weather and pleasant office environment.
